Below is a list of the items you will need to enroll in any of the programs at our school:
  1. Parent(s) or Guardian must be present
  2. Appropriate Application must be filled out
  3. Appropriate Academic Records
  4. Discipline and Intervention Records (where applicable)
  5. Suspension/Expulsion Hearing Form (where applicable)
  6. Withdrawal/Transfer Form (complete with points and attendance)
  7. 504 Documents (where applicable)
  8. IEP (where applicable)
    • Re-evaluation indicating more restrictive environment
    • Behavior Intervention Plan
    • Accomodations Form
    • Exceptionality information
    • Current school year suspension days
  9. Health Card
  10. Birth Certificate
  11. Social Security Card
  12. Intake Appointment
  13. Medication Form
